On one side of the page ask one of your maths teacher to write a recommendation letter for you regarding your aptitude in Mathematics. On the other side of the page give the following information and get it attested by the Head of the department / Principal of your college (do not attach any photocopies of mark sheets).

Participants will be provided with reading material, lunch, refreshments and stationery during the course of the Workshop. Out station participants will be paid second class train fare by shortest route, and efforts will be made to provide hostel accommodation for them.
